How do we prevent runaway agent loops?

Per-agent budgets are mandatory: max steps, max wall-clock time, max tokens, max tool calls per task. Add LLM-based loop-detection heuristics that break on step-repetition. Absence of budgets is the leading cause of "runaway agent" incidents in production.
